CONSENT-PERMISSION FOR THE PROCESSING OF PERSONAL DATA
By signing this consent-permission I, the owner of personal data (hereinafter the Client and/or the Owner of personal data), give my consent-permission for:
the processing by the Limited Liability Company “ESKA CAPITAL”, EDRPOU code 36258745 (hereinafter LLC “ESKA CAPITAL” and/or the “Personal Data Controller”) of personal data – any information relating to me, including, but not limited to, information about surname, first name, patronymic, passport data, identification code, date and place of birth, citizenship, address of residence and registration, family, social and property status, education, profession, income, contact phone/fax numbers, email address, etc. (hereinafter “Personal Data”) of the Personal Data Owner for the purpose of:
1) carrying out by the personal data controller its business activities, offering the full range of services of LLC “ESKA CAPITAL” and/or third parties – any persons with whom LLC “ESKA CAPITAL” is in contractual relations (hereinafter “Third Parties”), including by making direct contacts with the Owner of Personal Data by means of communication and/or providing services by LLC “ESKA CAPITAL” and Third Parties, including but not limited to conclusion, amendment, termination and/or performance of any contracts concluded with LLC “ESKA CAPITAL” and/or Third Parties and/or in connection therewith;
2) provision by Third Parties of services to LLC “ESKA CAPITAL” for the performance of its functions and/or for the performance of any contracts concluded by LLC “ESKA CAPITAL” with Third Parties;
3) protection by LLC “ESKA CAPITAL” of its legitimate rights and interests, including the transfer of data to any financial institutions (companies), legal advisers, third parties;
4) exercise of rights and fulfillment of obligations under other contractual relations between LLC “ESKA CAPITAL” and the Client: transfer (dissemination) of Personal Data to Third Parties, change, destruction of Personal Data or restriction of access to them, inclusion of Personal Data in the Personal Data database of LLC “ESKA CAPITAL” for the purpose specified in clauses 1-4 of this Consent-permission and without the need to provide the Personal Data Owner with a written notice of the performance of the specified actions.
Processing and storage of Personal Data is carried out for the period necessary in accordance with the purpose of processing of Personal Data provided by this Consent, but in any case until the termination of LLC “ESKA CAPITAL” and/or its successors.
At the same time, the term “processing of personal data” should be understood as any action or set of actions carried out in whole or in part in an information (automated) system and/or in personal data files, which are related to collection, systematization, registration, accumulation, storage, adaptation, modification, renewal, use and dissemination (distribution, realization, transfer), depersonalization, destruction of information about the Client, the controller of which is LLC “ESKA CAPITAL” and/or Third Parties.
By signing this Consent, the Personal Data Owner also confirms that he has been notified in writing about the personal data controller, about the composition and content of the Personal Data of the Client collected by LLC “ESKA CAPITAL”, about inclusion of Personal Data in the personal data database of LLC “ESKA CAPITAL”, about the rights provided by the Law of Ukraine “On Personal Data Protection” No. 2297-VI dated 01.06.2010, about the purpose of data collection and persons to whom the Personal Data are transferred, and certifies that the composition and content of the Personal Data correspond to the above-defined purpose of processing of Personal Data.
